The book: Game Change indicates the McCain campaign did a poor job of vetting Sarah Palin and Palin's husband, Todd was not even interviewed.
When McCain was asked if Palin was properly vetted, all he could say was that he didn't know.
The moral of this story is simple:
Piss poor planning produces piss poor results.
